Welcome to AQUA MD Mobile IV!

1. Payment

a. Payment for all services is due in full after the service, unless otherwise agreed to in writing in advance. We do not store credit card or other payment information on file. Clients are responsible for ensuring that an accepted form of payment is available at the time of their appointment. Failure to provide payment at the time of service may result in cancellation of the appointment and/or refusal of future services until the outstanding balance is resolved.

2. Treatment Refund and Dissatisfaction Policy

a. All services are non-refundable. Company does not issue refunds based on general dissatisfaction; however, at Company’s sole discretion, an adjustment, credit, or limited refund may be offered depending on the circumstances.


b. All refund or dissatisfaction-related requests must be made in writing within five (5) business days of treatment and are subject to evaluation on a case-by-case basis. Company reserves the right to deny refunds or adjustments if it determines, in its sole discretion, that the request falls outside the scope of this policy.

3. Guests and Children Policy

a. To maintain a safe and comfortable environment for all clients and staff, we kindly request that clients ensure pets are secured and children have an alternative adult supervising them during the IV treatment. Please make appropriate childcare arrangements prior to your visit.

4. Privacy Policy

a. We respect your privacy and are committed to protecting the confidentiality of your medical and personal information in accordance with all applicable laws. As part of our compliance with federal and state privacy regulations, including the Health Insurance Portability and Accountability Act (HIPAA), you may be required to sign additional HIPAA disclosures or consent forms when appropriate to ensure proper documentation and authorization.


b. We do not share your information with third parties except as required by law or as necessary to carry out treatment, process payments, or conduct essential healthcare operations. Your information will be handled with the utmost care and used only for legitimate purposes directly related to your care and our services.

5. Pre- and Post- Treatment Care Policy

a. To help ensure the safety and effectiveness of your treatment, you are required to follow all pre- and post-procedure instructions provided by your provider. These guidelines are designed to minimize risks and optimize your results. Failure to comply with the recommended care instructions may lead to suboptimal outcomes, complications, or the need for additional corrective treatment.


b. You acknowledge and agree that failure to comply with pre- or post-treatment instructions may increase the risk of complications, delay healing, diminish or negate the effectiveness of treatment, or necessitate additional corrective or follow-up care at your own expense.


c. Company and its providers are not responsible for any adverse effects, lack of results, or complications that arise from your failure to: (i) disclose complete and accurate health information during intake or consultation; (ii) follow prescribed pre- or post-treatment protocols; or (iii) attend required follow-up appointments or testing. Certain treatments may require ongoing monitoring, diagnostic testing, or lifestyle modifications as part of safe medical management (e.g., hormone replacement therapy, weight loss programs, or peptide protocols). You agree to comply with all monitoring requirements and understand that failure to do so may result in modification, suspension, or termination of your treatment plan at the provider’s discretion.


6. Medical Eligibility & Disclosure Policy


a. Clients must provide complete and accurate health history, including all medical conditions, medications, allergies, and prior procedures, as part of their intake and consultation. Clients agree to promptly disclose any changes to their health status throughout the course of treatment.


b. Company reserves the right to postpone, modify, or decline treatment if it determines, in its sole discretion, that a treatment is medically inappropriate, contraindicated, or unsafe due to incomplete disclosure or changes in medical condition.


c. Failure to provide accurate or complete information, or withholding relevant details, releases Company and its providers from liability for any adverse outcomes.


7. Artificial Intelligence (AI) Policy


a. Company may use artificial intelligence (“AI”) tools in connection with its operations, including scheduling, marketing, client communications, and other business functions. These tools are designed to improve efficiency and enhance the services we provide, but they are not perfect.

Although Company will take reasonable steps to safeguard your information, the use of AI may involve risks that are outside of Company’s direct control, including errors in processing or the unintentional disclosure of information. By receiving services, you acknowledge and accept these risks and agree that Company is not responsible for any damages, losses, or unauthorized disclosures that may arise from the use of AI in its operations.

8. Zero Tolerance

a. We are committed to maintaining a safe, respectful, and professional environment for both our clients and staff. Any form of harassment, abusive language or behavior, intimidation, or violence will not be tolerated under any circumstances. All individuals are expected to treat others with courtesy and respect while on the premises.


b. Company reserves the right to refuse service immediately if this policy is violated. This applies to conduct directed at staff, other clients, or anyone present at the facility. Continued or serious violations may result in a permanent ban from receiving services at Company.

9. Policy Updates

a. These policies are subject to change at any time without prior notice. We regularly review and update our procedures to reflect changes in the law, improvements to our practice, and the evolving needs of our clients. The most current version of our policies will be made available at our facility and published on our website. Clients are responsible for reviewing the most up-to-date version prior to receiving services, and continued use of our services constitutes acceptance of any updated terms.
